Antibiotic use tied to Crohn's, ulcerative colitis

NEW YORK (Reuters Health) - People who are prescribed a large number of antibiotics tend to have a higher risk of inflammatory bowel disease (IBD), a new study finds, providing more evidence that antibiotics may be disturbing bacteria in our intestine.
"It's not that antibiotics cause inflammatory bowel disease, but that it further supports the hypothesis that changing the gut flora may be disadvantageous," co-author Dr. Charles N. Bernstein, who studies bowel disorders at the University of Manitoba, told Reuters Health.
Previous studies have linked antibiotic use and IBD, which includes Crohn's disease and ulcerative colitis. In the current study, Canadian researchers found 12 percent of people diagnosed with the two conditions had been prescribed three or more antibiotics two years before compared to seven percent without the disease. This difference was consistent over a five-year period.
In other words, if antibiotics were solely responsible for the difference, for every 20 people prescribed three or more antibiotics, there would be one extra case of IBD.
Once the researchers took other factors into account, they found that people prescribed lots of antibiotics were as much as 50 percent more likely to get Crohn's disease within two to five years.
Published in the American Journal of Gastroenterology, the study looked at 24,000 people from the one of the largest IBD databases in North America.
"It's a well-done study that has a strong well-maintained database that allows the authors to get quality data," said Dr. Jean-Paul Achkar, who studies the genetics of IBD at the Cleveland Clinic and was not part of the study.
"They even tried to adjust for non-antibiotic prescription history, and still antibiotics came out as being associated with increased risk of IBD," he told Reuters Health.
An estimated 1.4 million Americans suffer from IBD, with approximately 30,000 new cases diagnosed each year, according to the Crohn's and Colitis Foundation.
Both Crohn's disease and ulcerative colitis - the two main forms of IBD -- cause chronic inflammation in the intestines, triggering symptoms like abdominal pain, diarrhea and weight loss.
The cause of IBD is unclear, but some scientists think IBD may result from the immune system overreacting to viruses or bacteria in the intestine.
"There is growing evidence that alterations in the balance of normal intestinal bacteria could lead to the development of IBD," said Achkar.
Most people who've taken antibiotics will not develop IBD. But Bernstein still thinks a portion of the population is at risk.
"It's not that we should stop using antibiotics," he said. "We get through a lot of important infections by using them. But we have to make sure that when we use them, it's for the right reasons."

Misuse of antibiotics has led to a global health threat: the rise of dangerous—or even fatal—superbugs. Methicillin-resistant Staphylococcus aureus (MRSA) is now attacking both patients in hospitals and also in the community and a deadly new multi-drug resistant bacteria called carbapenem-resistant Klebsiella pneumoniae, or CRKP is now in the headlines. DALLAS, April 6 (UPI) -- A study of Veterans Administration's acute care facilities indicates increased use of carbapenems, a powerful class of antibiotics, U.S. researchers say.On a similar topic of increased use of drugs at the VA and in civilian medicine, this is worth your consideration as well -
Dr. Makoto Jones and colleagues used barcode medication administration data for antibiotics administered in 110 VA acute care facilities from 2005 to 2009.
In the five-year study period, there was a gradual increase in overall antibiotic use and a 102 percent increase in the use of carbapenems -- a powerful class of "last resort" antibiotics. The study found a 79 percent increase in the use of intravenous vancomycin and 41 percent increase combinations of penicillin with beta-lactamase-inhibitors. A 20 percent increase in fluoroquinolones* were the most frequently used drugs across facilities.
Additionally, the researchers noted the quantity of antibiotics reported from VA facilities seems similar to reported data from non-VA hospitals in the United States.
"Use of these antibiotics helps the patient receiving the treatment, but has future consequences for innocent bystanders," Jones says in a statement.
"The more these drugs are used, the more resistance we see."
The study was presented at the annual meeting of the Society for Healthcare Epidemiology of America in Dallas.

Decades after spawning a health care revolution, antibiotics are now common pollutants. Scientists' biggest concern about these emerging contaminants is that they promote the spread of resistance. But new research suggests they also harm the microbes that cleanse groundwater of dangerous compounds, particularly nitrates (Environ. Sci. Technol., DOI: 10.1021/es103605e).
High nitrate levels in drinking water can cause methemoglobinemia, a disease that decreases the blood's oxygen carrying capacity. Naturally-occurring bacteria in groundwater, such as Pseudomonas putida, can remove nitrates by reducing them to nitrogen gas.In Cape Cod, Mass., very high nitrate levels co-occur in groundwater with one of the most common antibiotics in the clinical arsenal: sulfamethoxazole (SMX). For nearly a decade, microbiologist Ronald Harvey and colleagues from the U.S. Geological Survey have tracked SMX and other groundwater pollutants at an aquifer that originates at the Otis Air National Guard Base, a heavily polluted site on Cape Cod. Other researchers had shown that high doses of SMX can interfere with bacterial nitrate reduction. But no one knew if a similar response might occur at environmentally-relevant concentrations.
To answer that question, Harvey's team first cultured bacteria from a non-contaminated portion of the aquifer. Next, they added nitrate to the cultures at levels measured in the environment, along with SMX at doses ranging from 0.005 to 2,000 µM. Bacterial growth rates dropped at all doses. At the environmentally relevant concentration of 0.005 µM SMX, the amount of total nitrate that the bacteria removed from the culture fell by nearly half. "We're demonstrating a clear biological effect," Harvey says. "And we're showing that in the same bacteria that live in this particular aquifer."
Harvey says that the next step would be to study the bacteria's response to SMX exposure in the aquifer itself.
USGS research hydrologist Dana Kolpin, who didn't participate in this study, thinks the results suggest that SMX contamination might account at least in part for high nitrate levels in the aquifer. "We can't jump to that conclusion yet," he says. "But the data suggest that's a hypothesis worth pursuing."

Homeopathic Treatment of Otitis Media: A Literature Review

In an observational study, Frei and Thurneysen4 sought to determine how many children with acute otitis media were relieved of pain with individualized homeopathic treatment. Children with this condition received a first individualized homeopathic medicine in the pediatric office. If pain reduction was not sufficient after 6 hours, a second (different) homeopathic medicine was given. After a further 6 hours, children who had not experienced pain control were started on antibiotics. Pain control was achieved in 39% of the patients after 6 hours and in another 33% after 12 hours. Compared with literature data the researchers had consulted, they stated that the resolution rate was 2.4 times faster than in untreated cases. The six most frequently prescribed remedies were Pulsatilla, Belladonna, Sulphur, Phosphorus, Calcium Carbonicum, and Lycopodium. An interesting multicenter, prospective, observational study in a real-world medical setting compared the effectiveness of homeopathy with conventional medicine.5 Thirty investigators (Riley et al.) with conventional medical licenses at six clinical sites in four countries enrolled a series of patients with at least one of the following three complaints: upper respiratory tract complaints including allergies; lower respiratory tract…Homeopathy is a 200-year-old system of medicine used successfully by many over 100 million patients worldwide. There are many natural treatments for ear ache ranging from Auralgan to garlic oil to using the child's urine as a homeopathic remedy.
Why risk their health along with gastrointestinal problems related to anti biotic use, or weakened immunity from the overload of vaccines that lately don't seem to be effective at all, or at least that's what the scientists are saying.
Here's some help and remember that you can make a very good remedy of your own with organic garlic and olive oil.
The treatment is only modestly effective and reduces the period of infection only a small amount, but it helps relapses and progression of the ailment among patients under age 2, two new trials indicate.
The medical consensus on whether to give antibiotics to young children with ear infections has been swinging from one extreme to the other as conflicting clinical trials have pushed pediatricians first toward widespread use of the drugs, then toward a "watch and wait" approach in which most infections seem to clear up on their own.
Two new trials reported Wednesday in the New England Journal of Medicine are nudging the pendulum back toward treatment of the infections, especially for the youngest children. They show that the use of antibiotics in infants and toddlers under the age of 2 is only modestly effective and reduces the duration of symptoms by only a small amount, but it prevents relapses and progression of the disease.

In this occasional series, Anahad O’Connor, the New York Times “Really?” columnist, explores the claims and the science behind alternative remedies that you may want to consider for your family medicine cabinet.The Remedy: Herbal ear drops.The Claim: They relieve pain from ear infections.The Science: As many parents know, ear infections can be one of the most painful — and common — conditions that afflict children. Millions of cases are diagnosed every year, and some studies show that about 90 percent of all children in the United States experience at least one episode of acute otitis media, as the condition is known scientifically.But treatment is controversial. Many doctors write antibiotic prescriptions even though medical experts say they do almost nothing to help, and in some cases may do more harm than good.Depending on a child’s age and symptoms, medical experts advise parents to take a wait-and-see approach, in part because almost two-thirds of children recover from pain and fever within 24 hours of diagnosis. In the meantime, some doctors prescribe anesthetic medications like antipyrine combined with benzocaine to help ease the pain, but studies suggest that herbal ear drops may be just as effective, with minimal side effects.In one study, published in The Archives of Pediatrics and Adolescent Medicine in 2001, a team of pediatricians recruited 103 children ages 6 to 18 with earaches resulting from acute otitis media. The subjects were randomly split into groups and assigned to be treated with either anesthetic ear drops or an herbal ear drop known as Otikon, which contains extracts of antibacterial and anti-inflammatory agents like garlic, calendula, St. John’s wort and mullein (a flowering plant) in an olive oil solution. At the end of the study, the researchers found reductions in ear pain in both groups, and concluded that the herbal ear drop was just as effective as the anesthetic one.A similar study appeared in 2003 in the well-respected journal Pediatrics. This time, the scientists used an herbal ear drop that contained the same ingredients as in the earlier study, as well as lavender and vitamin E. The scientists recruited about 180 children ages 5 to 18, then randomly assigned them to numerous groups. Some of the children received the herbal ear drops alone (three times a day, five drops at a time), and others received them in combination with the antibiotic amoxicillin. Other children were treated with anesthetic ear drops, with or without antibiotics.Ultimately, the patients who were given the ear drops alone had greater pain relief than those who took them with the antibiotic. The scientists concluded that in many cases, the pain caused by acute otitis media may subside with the simple passage of time. But in cases where “active treatment” is needed for pain relief, an herbal ear drop may be just as good as one that requires a prescription.The Risks: In the studies, no side effects were noted. But parents should always consult with a doctor before deciding on a course of treatment.
